We are so grateful you have decided to join us for this special festival running June 23 – June 26, 2022, and we can’t wait to get the process rolling!
Below are the documents we need you to complete before arriving for the festival. Please note that some of these forms have different due dates. Why are these forms separate? We recognize that you will know some information (like transportation) sooner than you will know other information (like final room assignments) and can not submit everything at once.

General Choir & Repertoire Information:
Due April 1, 2022 – CLICK HERE to fill out form.
This form collects your choir & repertoire details, which will be used when creating our program materials for the concert. Please note: your choir will wear their performance uniforms to perform their repertoire. They will then change into our Queen City festival T-Shirt, keeping on their choir’s uniform bottoms, for the combined rep performance.
Rooming List Template:

Complete your rooming list, and email your saved file to Rachel Breeden upon completion.
Please note: This document will serve as your final head count for the event, and updated invoices will be created at this time for your choir if there are any changes from the original invoice. Please Quad occupancy rooms available for this festival. We will have limited availability double and single rooms for an additional charge of $50 per person/ double occupancy and $100 per single occupancy room requested. Garage parking passes are also available if needed, and can be secured by noting “1” next to the person who will be using the pass.
Meal & T-Shirt Information:

Please let us know about any dietary restrictions (or acknowledge that you have none in your choir) by completing this form. Please also use this form to indicate the t-shirt sizes you will need so that each participant in your choir receives a commemorative festival shirt (worn during combined festival rep. performance).
Full Festival Payment:
Due May 1, 2022 – No refund of payment will be made after this date.

COVID Policy & Waiver/Medical Emergency/ Media Release Form:
Due upon arrival – CLICK HERE to download our festival COVID Policy & Waiver/Medical Emergency/Media Release Form
This form contains our COVID Policy & Waiver, Emergency Medical, and Media Release details. You will need a form for each singer in your choir. Upon arrival you will acknowledge that you have your forms. You will keep your forms with you during festival, and then turn them in to CYC at the end of the festival. CYC will keep the forms for 2 months, and then they will be destroyed to protect participants’ privacy.
